Complete GIW's credit application online or download it as a PDF file, which you can complete offline and fax to GIW.

If you complete the application online, your information is encrypted through our secure server. You can verify this by looking for a closed lock icon at the bottom of the new window that will open when you click on the link to the online credit application above.

After you complete the online application, you will recieve an email confirming that we received your application. However, we cannot process your application without the following documents:

If your company:

• Is a tax-exempt U.S. company, please fax a copy of your company's sales tax exemption certificate for each U.S. state to which your company will be shipping; this requirement does not apply if your company is a foreign (non-U.S.) company that directly imports GIW's products into your country.
• Does not have a Dun & Bradstreet rating, fax a copy of your latest financial balance sheet, profit & loss and cash flow statements.
