Use this form to request returning goods to GIW.
Fields in bold are required. Items in red are required. Please try to provide as many of these items as possible. A minimum of two "red" items is required to submit this form.
If you don't know the preferred carrier (collect), call giw traffic manager or technical services at 706.863.1011, Ext. 2362, or Ext. 2383, for the carrier name.
After you submit the form, you will be contacted by GIW with a Returned Goods (RG) number.
Credit will only be issued if:

- Warranty terms apply.
- The RG Number is legible on each returned good.
- The returned parts pass GIW's Quality Control inspection.
- Domestic returns are received by GIW within 30 days of the RG Number's issuance.
- International returns are received by GIW within 75 days of the RG Number's issuance.
